MOSAIC: Bringing Precision Heart Care to All

Overview

The MOSAIC Study is funded through the Canadian Precision Health Initiative (CPHI), a national program dedicated to bringing genomics, data, and AI into healthcare to deliver more personalized and equitable care for Canadians. MOSAIC focuses on cardiovascular health — the leading cause of death in Canada — and aims to create a model for how precision health can be scaled and delivered equitably across diverse populations.

MOSAIC integrates whole-genome sequencing, echocardiography, and clinical data to uncover new markers of heart disease and guide better prevention and treatment strategies. Importantly, the study is designed to address gaps in access by ensuring that rural, remote, and Indigenous communities are included in the benefits of genomic medicine.

By aligning with the Pan-Canadian Genome Library (PCGL), MOSAIC also ensures that the data and insights generated will contribute to Canada’s national genomic resources, while building a framework for how local discoveries can scale to national impact.

Together, MOSAIC and CPHI are laying the groundwork for a future where precision cardiovascular care is accessible, equitable, and embedded in the health system.

Technology

MOSAIC brings together a powerful combination of technologies to create one of the most comprehensive pictures of cardiovascular health ever assembled in Canada. By integrating multiple data sources, we can detect early signs of disease, uncover new biological markers, and develop tools that guide personalized care.

Echocardiography (Echo)

  • Standardized cardiac ultrasound imaging to assess heart structure and function.
  • Includes point-of-care ultrasound (POCUS) to extend access to rural and remote settings.

Genomics

  • Whole-genome sequencing (WGS):
  • Short-read sequencing for broad coverage and variant detection.
  • Long-read sequencing for structural variants and difficult-to-map regions.
  • Provides a genomic foundation for building virtual disease panels on a single backbone.

Blood-Based Biomarkers

  • Clinical chemistry tests integrated with advanced molecular profiling.
  • Supports risk stratification, disease monitoring, and therapeutic targeting.

Multi-Omics Integration

  • Proteomics – protein-level markers of disease pathways.
  • Lipidomics – insights into cardiovascular risk through lipid profiles.
  • Metabolomics – systemic signatures of heart and vascular health.

Comprehensive Health Data

  • Linked electronic health records, survey data, and imaging results.
  • Provides real-world context to interpret genomic and biomarker findings.
